Successful applicants will support residents and families while maintaining health, hygiene, safety, empathy, teamwork, and person-centred care standards. Provide clinical care to aged-care residents, complete assessments and individualized care plans, maintain accurate clinical documentation including AN-ACC and Commonwealth Standards, and supervise Personal Care Workers. Build rapport with residents, families, and visitors while delivering empathetic, person-centered care. The role requires a nursing diploma, current AHPRA registration, aged-care experience, and strong documentation and computer skills.

Create and coordinate social, emotional, and spiritual lifestyle programs for aged-care residents. Build relationships with residents and families, coordinate activities inside and outside the home, establish lifestyle goals, implement supportive programs, and collaborate with the wider care team to meet residents’ needs.

Provides person-centered support to aged-care residents, including assistance with daily living, nutrition, comfort, activity, challenging needs, treatments, and health monitoring.
