At Mission Australia, we believe everyone deserves the opportunity to thrive. As a Case Manager, you'll work alongside people experiencing complex challenges, providing person-centred support that empowers them to achieve their goals, build independence and create lasting positive change.

Using a strengths-based and trauma-informed approach, you'll develop meaningful relationships with clients, coordinate tailored support plans and collaborate with community partners to deliver high-quality outcomes.

We are currently recruiting 3 full time role and one part time permanent roles.

Your key responsibilities will be to:

  • Deliver comprehensive, person-centred case management for individuals experiencing homelessness or at a risk of homelessness with complex needs
  • Undertake client assessments, develop holistic case plans and review progress towards agreed goals
  • Coordinate referrals and connect clients with appropriate internal and external support services
  • Provide evidence-based interventions that promote independence, wellbeing and positive outcomes
  • Support clients through transitions, helping them develop the skills and confidence to self-manage and access ongoing supports
  • Build strong partnerships with community organisations, government agencies and service providers to enhance client outcomes
  • Facilitate group programs, education sessions or outreach activities where required
  • Maintain accurate case notes, client records and reporting in line with organisational and funding requirements
  • Contribute to continuous improvement initiatives while supporting a collaborative and inclusive team environment
  • Provide mentoring and guidance to colleagues where appropriate

Requirements for Success

  • Relevant tertiary qualifications in Social Work, Community Services, Human Services or a related discipline or equivalent experience
  • Experience providing case management to people with complex needs within the community services, housing, homelessness, mental health or related sectors
  • Strong assessment, planning and case coordination skills
  • A sound understanding of strengths-based, trauma-informed and person-centred practice
  • Excellent communication, relationship-building and stakeholder engagement skills
  • The ability to manage competing priorities while maintaining high-quality documentation and reporting
  • A commitment to empowering people to achieve sustainable outcomes

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
