About HammondCare

HammondCare provides expert aged care, home care, dementia care and palliative care for older people, regardless of circumstances, beliefs, background or financial situation. We are led by our not-for-profit, Christian foundations and backed by research and clinical expertise.

When it comes to our people – those we serve and those who serve – we are committed to inclusion across race, culture, ethnicity, gender, sex, sexuality, religion and ability. In our innovative residences, our hospitals, or in client homes, we aim to set the global standard of relationship-based care for older people with complex needs.

We’ve been caring for people for over 90 years – so, if you love to care, join us, and make a career move that matters.

It's not just care, it's HammondCare.

About the role

As a Physiotherapist you’ll be a valued member of our growing Home Care Clinical Care & Practice Team, working closely with an experienced, multidisciplinary team to provide tailored and individualised services to older adults living at home. A typical day will involve assessing functional care needs, setting care goals, and providing treatment and intervention, helping people stay active, independent and right where they belong – home.

You’ll be provided with a broad and varied caseload, with support from experienced clinicians. This meaningful role will include performing home visits or running group activities for clients accessing Support at Home or CHSP services or residing in one of our Seniors Living units. You will deliver person-centred care in an environment that promotes continuous quality improvement, consistent with the values of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).

This is a permanent, full-time role based in the South West Sydney region, with an office located at Hammondville.

How we care for you while you care for others

  • Additional leave purchase options and 14 weeks paid parental leave
  • Flexible working options, including hybrid arrangements
  • Discounts on everyday products and services
  • Discounted healthcare memberships – access to a network of fitness centres and pools for employees and families
  • Free access to independent counselling support for employees and families
  • Health & Wellbeing programs
  • Reward & Recognition programs

  • Career and professional development opportunities

About you

  • You’re a tertiary qualified Physiotherapist with current AHPRA registration and practical experience, or sound knowledge in Aged Care, Inpatient, Community, Home-Based or Rehabilitation or related setting.
  • You're a strong communicator, and able to plan and manage your time effectively.
  • You enjoy working independently as part of a multidisciplinary team.
  • You have access to a reliable vehicle, valid driver’s license and safe driving record to travel between client’s homes and HammondCare locations (travel costs reimbursed).
