  • Implementation & Adoption Support: Coordinating activities that help teams transition to new processes, systems and ways of working and supporting adoption after go-live.

  • Adoption Monitoring & Reinforcement: Measuring adoption outcomes, identifying barriers to sustained transformation and working with leaders to address issues impacting uptake.

  • Enterprise Transformation Visibility: Maintaining an organisation-wide view of transformation activity to identify cumulative impacts, competing demands and stakeholder risks across HammondCare.

  • Continuous Improvement: Capturing lessons learned and using insights from implementation and adoption activities to strengthen future transformation approaches and support better outcomes.
